Context of Exodus 19:18
16On the morning of the second day, there was thunder and lightning with a heavy cloud over the mountain, and a very loud blast from a ram’s horn ⌞was heard⌟. All the people in the camp shook with fear. 17Then Moses led the people out of the camp to meet with God, and they stood at the foot of the mountain. All of Mount Sinai was covered with smoke because the Lord had come down on it in fire. Smoke rose from the mountain like the smoke from a kiln, and the whole mountain shook violently. 19As the sound of the horn grew louder and louder, Moses was speaking, and the voice of God answered him. 20The Lord came down on top of Mount Sinai and called Moses to the top of the mountain. So Moses went up.

- Matthew Henry Concise Commentary · v. 16-25Verses 16-25 Never was there such a sermon preached, before or since, as this which was preached to the church in the wilderness. It might be supposed that the terrors would have checked presumption and curiosity in the people; but the hard heart of an unawakened sinner can trifle with the most terrible threatenings and judgments. In drawing near to God, we must never forget his holiness and greatness, nor our own meanness and pollution. We cannot stand…Read the comment

Some of the experiences were visual: lightning and a cloud (19:16), smoke and fire (19:18). The people also heard thunder, a ram’s horn, and the voice of God (19:16, 19). The entire mountain shook violently (19:18). In the Bible, such unusual events are often associated with a theophany, which is a visible appearance of God’s holy presence.Read the comment
