Context of Psalms 38:19
17For I said: “Don’t let them gloat over me or exalt themselves over me, when my foot slips.” 18For I am about to fall, and my pain is before me constantly. So I confess my guilt. I am troubled because of my sin. 20My lively enemies are numerous. Many hate me wrongfully. 21Those who repay evil for good oppose me because I pursue what is good.

- Barnes’ Notes on the Bible · v. 19But mine enemies are lively - DeWette renders this, “My enemies live and are strong.” The word translated “lively” - חיים chayiym - means properly “living, being alive.” The literal translation would be, “My enemies, being alive, are strong.” The idea is, that while he was weak and apparently near to death, they were in the full vigor of life and health.
